Metformin was developed for type 2 diabetes, where it works mainly by improving insulin sensitivity. That mechanism is why researchers keep asking what else it might help with — but it's worth separating the established from the investigational.

Weight and metabolism (established)

Metformin is a gentle contributor to weight management, especially in people with insulin resistance. In the long-term Diabetes Prevention Program data, participants saw an average of roughly 2% body-weight reduction — modest, and best understood as a complement to lifestyle change rather than a primary weight-loss drug.

Aging and longevity (emerging)

Metformin draws real interest in the longevity field for its effects on cellular metabolism and oxidative stress. But this is an area of active, unsettled research — the honest framing is 'being studied,' not 'proven to slow aging.' Any provider promising the latter is getting ahead of the evidence.

Considerations

The most common side effects are mild and gastrointestinal, usually easing over time; starting low-dose and taking it with food helps. It requires a provider's evaluation, particularly for anyone with kidney or liver concerns, and periodic monitoring.
