Consent to Telehealth Services

What telehealth is
Telehealth (or telemedicine) is the use of secure electronic communications — such as messaging, questionnaires, phone, or video — to deliver healthcare services when you and the provider are in different locations. Holistically Rx uses telehealth to connect you with independent, licensed providers for weight, hormone, longevity, and diagnostic services.
This document explains telehealth so you can give informed consent before receiving services. It is provided for transparency; the provider who evaluates you may also confirm your consent at the point of care.
Nature of the services
During a telehealth encounter, an independent licensed provider reviews the information you submit — which may include your assessment answers, health history, and any laboratory results — and communicates with you electronically. A traditional in-person physical examination is generally not part of a telehealth visit.
Whether any treatment, medication, or testing is appropriate is decided solely by the independent licensed provider based on their clinical judgment. There is no guarantee that a prescription or any particular treatment will be issued.

- More convenient scheduling and follow-up, without waiting rooms or travel.
- Timely coordination of prescriptions and laboratory testing where appropriate.

Potential risks and limitations
As with any medical service, telehealth has limitations and potential risks, including but not limited to:
- The provider may determine that your condition cannot be adequately evaluated by telehealth and may recommend an in-person visit or decline to provide services.
- Technical problems — such as poor connectivity or equipment failure — may delay or disrupt the consultation.
- In rare cases, information transmitted electronically may be insufficient for sound clinical decision-making, or security protocols could fail despite reasonable safeguards.
- Delays in evaluation or treatment could occur due to technology or scheduling.
Your rights
Your participation in telehealth is voluntary. You have the right to withhold or withdraw your consent to telehealth at any time without affecting your right to future care or treatment. You have the right to ask questions about the services, and to request that a consultation be discontinued.
You understand that no specific outcome, prescription, or result is guaranteed, and that any treatment is provided only when an independent licensed provider determines it is clinically appropriate for you.
Privacy and confidentiality
The laws that protect the privacy and confidentiality of medical information also apply to telehealth. Information disclosed during your consultation is protected under applicable federal and state law, including HIPAA. Acknowledgment and consent
By using Holistically Rx's services and proceeding to a consultation, you acknowledge that you have read and understood this Consent to Telehealth Services, that your questions have been (or may be) answered by your provider, and that you consent to receive services via telehealth under these terms.
